Atrial Fibrilation in Practice: Gregory YH Lip

چکیده

These two volumes form part of the excellent In Practice series, published by the Royal Society of Medicine. Each examines an aspect of cardiology and presents it in such a way as to be accessible for the expert and the generalist alike. Professor Lip is well known for his work on atrial fibrillation, and the book manages to include a lot of information. Despite this, it is very readable. My only concern is that there is distinct trans-Atlantic flavour to the prescribing. No local GP will prescribe Class 1 antiarrhythmic drugs without specialist advice, and so a detailed discussion of them is out of place. In addition, dofetilide is not licensed in the UK. 1 feel the author underestimates the long-term effects of amiodarone on the lungs, and serial measurements of diffusion capacity would be a more sensitive way of detecting these than chest X-rays. Finally, I would have thought that a mention ofthe forthcoming new anti-thrombin drugs would be appropriate, as warfarin and all its problems could soon be a thing of the past!

Foreword Lane & Lip Advances in therapy for atrial fibrillation Foreword

Two epidemiological studies have demonstrated that the lifetime prevalence of atrial fibrillation (AF) among people aged 40 years or more is roughly one in four [1,2]. AF increases in prevalence with age, ranging from 0.5% in people aged 50–59 years to almost 9% in those aged 80 years or more, and is associated with a high risk of morbidity and mortality from heart failure, stroke and thromboem...

Clinical review: Clinical management of atrial fibrillation – rate control versus rhythm control

Atrial fibrillation (AF) is the most common sustained cardiac arrhythmia in the critically ill and is associated with adverse outcomes. Although there are plausible benefits from conversion and maintenance of sinus rhythm (the so-called 'rhythm-control' strategy), recent randomized trials have failed to demonstrate the superiority of this approach over the rate-control strategy. Regardless of a...

Antithrombotic therapy in anticoagulated patients with atrial fibrillation presenting with acute coronary syndromes and/or undergoing percutaneous coronary intervention/stenting.

The management of antithrombotic therapy in atrial fibrillation patients presenting with acute coronary syndrome and/or undergoing percutaneous coronary inter vention/stenting cannot be done according to a regimented common protocol, and stroke and bleeding risk stratification schema should be employed to individualize treatment options. A delicate balance is needed between the prevention of th...

Edoxaban in venous thromboembolism and stroke prevention: an appraisal

Oral anticoagulation is the therapeutic cornerstone in preventing thromboembolic risk in both atrial fibrillation (AF) and venous thromboembolism (VTE). After decades of the sole therapeutic oral anticoagulation option being warfarin, the introduction of non-vitamin K antagonist oral anticoagulants has heralded a new era. Edoxaban is the latest addition to these available for clinical use. Edox...
